Annual Wellness Visit

Your annual preventive health exam (aka well visit or annual physical) is scheduled with your primary care physician to catch potential health issues early, before they become serious, and to help you focus on wellness and a healthy lifestyle while identifying important screening tests, vaccinations and other necessary testing. ​


What to expect BEFORE your visit:

  • Your personal physician will spend time reviewing your medical records prior to your visit.
  • You will receive a pre-visit questionnaire to complete online prior to your visit. These questionnaires are age specific and help to identify the most important topics to address during our time together.

What to expect DURING your visit:

  • Full vital signs including: blood pressure, pulse, respirations, O2 saturation, weight & height
  • Review of your past medical history, including:
    • medical and surgical history
    • medications and allergies
    • lifestyle (job, relationships, exercise habits, etc)
    • family history
    • immunization records
    • recent test results
  • Complete physical exam, which may include:
    • Hearing and vision screens
    • Pap smear (recommended every 3-5 years for most women)
    • Additional examinations may be recommended based on your past medical history or current symptoms.
  • Personalized recommendations may include:
    • vaccine recommendations
    • exercise advice
    • ​diet advice
    • referral for specialized screening tests (colonoscopy, mammogram, eye exam)

What tests are recommended during a routine physical?

Most adults are recommended to have the following lab tests done once a year:
  • CBC (Complete blood count) - screen for infection, anemia, bone marrow disorder
  • CMP (Complete metabolic panel) - screen for kidney function, liver function, electrolytes
  • TSH (Thyroid Stimulating Hormone) - screen for thyroid health
  • Hgb A1c (Hemoglobin A1c) - screen for diabetes
  • Vitamin D - screens for deficiency
  • Lipid panel - screen for cholesterol
  • ApoB (Apolipoprotein B) - screen for cardiovascular disease
  • LpPLA2 - screen for cardiovascular disease

Additional screening tests may include:
  • Urine microalbumin/creatinine ratio - screen for kidney disease
  • B12 level (especially if vegetarian, on a proton-pump inhibitor or taking Metformin)
  • Ferritin - screen for iron deficiency
  • Advanced cardiovascular labs
  • Genetic testing
  • EKG
  • ​and more

Are there any additional costs?

Your annual wellness visit is included with your membership. Hearing/vision screens & EKG provided when necessary - at no additional fee.

Labs - drawn in-house and charged to patients at our discounted rates. Costs will vary based on which labs are necessary, but generally run between $75-150, total cost*.

Pap smears - there is no additional fee for this procedure. Patients are charged only for our discounted pathology fee. *Cost is $50 (pap alone) to $75 (pap, including HPV testing).

We provide referrals for other routine screening tests, such as formal retinal eye exams, mammograms and colonoscopies. Since we are an independent medical practice with no restrictions or constraints of any particular health system, we can send you wherever we - you and your doc - deem best.
  • For patients who are cash pay, we have partnered with many great local specialists and imaging centers to help find the most cost effective options.
  • For patients using traditional insurance, these screening tests are generally required to be covered at zero out of pocket cost, but may have restrictions on location -please confirm the conditions of your policy directly with your insurance company before scheduling. ​
